North Shore Chronic Disease Services (NSCDS) offers Nutrition Counselling for North Shore residents of all ages.

We serve people of all ages who are North Shore residents.

Get a referral
A referral is required. We accept referrals from Physicians, Nurse Practitioners and dietitians. Have your Physician, Nurse Practitioner or Dietitian complete the North Shore Chronic Disease referral form and fax it to (604) 297-9681

- Monday: 8:30 a.m. to 4:30 p.m.
- Tuesday: 8:30 a.m. to 4:30 p.m.
- Wednesday: 8:30 a.m. to 4:30 p.m.
- Thursday: 8:30 a.m. to 4:30 p.m.
- Friday: 8:30 a.m. to 4:30 p.m.
- Saturday: Closed
- Sunday: Closed
Parking & Transportation
Underground parking is available, as well as limited street parking.
Elevators are available from the underground parking lot.
