This North Shore community-based program provides a general foot assessment, related education, footwear review and early interventions to prevent foot ulcers, wounds and lower-extremity amputations for people living with diabetes. Clients are typically seen by our foot care team for 1-4 visits over a one-year period. The foot care team consists of a Registered Nurse and an Occupational Therapist. This program is not a substitute for podiatry assessment and services.

How to access

  • Check eligibility

    We only accept referrals for individuals living with diabetes who currently reside in the North Shore community and are able to attend appointments at our location at West Vancouver Community Health Centre. We will not see clients with untreated and undressed wounds. Individuals with wounds need to be receiving wound care if they are also referred to the Diabetes Foot Care Program.

    Any health care provider or physician may refer. Have your health care provider complete the North Shore Chronic Disease Service referral form and fax the completed form to (604) 297-9681.

    This service is offered in English. Interpreter services are available for non-English speaking clients, usually by telephone.

The Foot Care Program is located on the 2nd floor in room 241.

There is free 3-hour underground parking or District street parking.
