HealthConnection provides primary care for North Shore residents with "high needs," defined as individuals who don?t have a regular family doctor (GP) or nurse practitioner (NP) and face complex medical, mental health and addictions, and/or socio-economic needs (like housing, income, and access to food).
Clients can drop in as self referrals, or be directed to the clinic by family physicians and health care providers within Vancouver Coastal Health (including the Lions Gate Hospital emergency department) and community agencies.
The clinic team includes a family physician, a nurse practitioner, a chronic disease nurse coordinator and a medical office assistant. In addition, the clinic is partnering with community agencies to support clients who require assistance with non-medical issues such as housing, income, and access to food.
This clinic is made possible through partnership between the North Shore Division of Family Practice and Vancouver Coastal Health.

Individuals can drop-in during all hours from Monday to Friday.
Appointments can be made on Monday, Wednesday and Friday afternoons.
Clients can be directed to the clinic by other health care providers or community service providers. No scheduled appointment or formal referral is necessary to be seen at HealthConnection Clinic.
?Individuals must not have a family doctor/primary care provider or must be willing to change and attach to the HealthConnection Clinic, if they meet complexity criteria.
