OsteoArthritis Service Integration System is a program where we connect people who have osteoarthritis, their families and health care providers with information, education and support to manage osteoarthritis at every stage of the disease.

Education

What to expect

After you are referred to OASIS, you will receive a phone call from our office. We will schedule an assessment appointment for you at the OASIS clinic of your choice. You may need to go for an x-ray before your assessment appointment if you have not had an x-ray within the last year.

In order to get the most out of your OASIS appointment, complete the client section of the assessment tool and bring it to your appointment. 

At the OASIS clinics our team, including physiotherapists, occupational therapists or nurses, will assess your condition and help you to identify goals to manage your osteoarthritis. Your appointment may take up to 1 or 1 ? hours.

After the appointment, you will receive a personalized Action Plan outlining recommendations for treatment, follow up and referrals to support services and community programs. We will work with you and your family doctor to carry out the recommendations of your Action Plan.

If you are a candidate for surgery, OASIS will assist your family doctor in referring you to a surgeon.

Eligibility

You can attend OASIS if you have osteoarthritis of the hip, knee, ankle, foot, wrist or hand.

Your osteoarthritis does not have to be severe in order to attend OASIS. Our clients range from those with early osteoarthritis to those with more severe osteoarthritis.

Is there a cost for the OASIS clinic?

OASIS is a publically funded program and assessments are provided free of charge.

OASIS is an assessment and referral program that works with physicians and other health care providers to improve the quality of life and health outcomes for people with osteoarthritis.

Primary care physicians, rheumatologists, surgeons and other health care providers helped develop the OASIS program and continue to be involved today.

How does OASIS benefit physicians?

OASIS assumes many of the administrative and education tasks necessary to support people with osteoarthritis so physicians can focus on client care.

  • OASIS provides primary care physicians with a comprehensive interdisciplinary assessment report for their patient and assists with coordination of action plans.

  • OASIS screens patients before they are assessed by a surgeon, provides the surgeon with a comprehensive assessment report and provides the patient with education to prepare for surgery.

Surgeons will find they spend less time completing forms and more time assessing appropriate surgical candidates.

What can primary care physicians expect from OASIS?

OASIS will keep primary care physicians informed of every step of their patient?s journey by:

  • Informing you of patient appointments

  • Providing assessment reports and action plans

  • Calling you if our clinicians have immediate concerns

  • Letting you know of surgical consultation requests,

  • Notifying you when patients attend education sessions ? both primary and surgical preparation

  • Sending reminders for patient follow-up with OASIS

Who does OASIS partner with?

We work with physicians, health care providers and independent organizations to identify and fill gaps in care and develop services to meet the needs of people with osteoarthritis.

For example, we partner with the Mary Pack Arthritis Center to share knowledge and  increase access to services. We work with local community centers to develop and refer to appropriate exercise programs.  We also work with community centres and The Arthritis Society to provide community education sessions.

Sharing knowledge and expertise

We value teamwork and sharing of knowledge and expertise between diverse disciplines and health agencies. We share program information, including lessons learned, through site visits, teleconferences, presentations and hosting education sessions for physicians and health providers.

OASIS is the first of its kind in British Columbia. 

A new model of care

Before OASIS, people with osteoarthritis did not have one service that could help to coordinate their care. People with osteoarthritis and their health care providers were unsure about where to find the most current information, programs and services for managing osteoarthritis.  Although services were available for people after they had surgery, there was very little information about preventing osteoarthritis, managing osteoarthritis conservatively and preparing for joint replacement surgery.

OASIS was developed to meet the unique needs of patients, families and health care providers dealing with osteoarthritis by providing information about:

  • Self-management

  • Prevention

  • Alternatives to surgery

  • Preparing for surgery

OASIS also provides access to:

  • Resources and support services

  • Services for people who do not speak English

  • Services for people dealing with multiple health concerns

OASIS saw its first client in 2006

The first OASIS clinic opened in September 2006 to referrals from a small number of family doctors and surgeons.  Tools and processes were tested over the following year and the clinic expanded to include referrals from family doctors, rheumatologists and surgeons in September 2007.  OASIS has grown from its first clinic in Vancouver to its present 3 locations in Vancouver, Richmond and the North Shore.
