Healthy Living Program - Vancouver Community

?The Healthy Living Program (HLP) provides services in various locations across Vancouver. 

HLP is a health promotion and chronic disease prevention and management program for adults living in Vancouver who have or who are at risk for developing chronic diseases.

We support communities to create  opportunities and overcome barriers in order to improve health for all by promoting:

  • Active living 
  • Healthy eating 
  • Wellness

We work with VCH staff, community leaders and agencies to develop initiatives and partnerships that promote healthy behaviour changes through our programs and services. Contact our office for information on upcoming courses.  A list of our programs and services are outlined in our Healthy Living Program brochure.

We help individuals make sustainable healthy lifestyle changes through the emphasis of key concepts such as:

  • Self-management
  • Goal setting
  • Action planning

Our team

Our interdisciplinary team includes professions such as Registered Nurses, Registered Dietitians, and certified Exercise Physiologists.

Eligibility

Healthy Living programs and services are offered to patients/clients and VCH staff. We also work with different neighborhoods and groups in Vancouver. Special consideration is given to adults living with health disparities including:

  • Adults with limited income

  • Adults who are socially isolated or lack social networks

  • Ethno-cultural groups

  • Aboriginal peoples
