HLP is a health promotion and chronic disease prevention and management program for adults living in Vancouver who have or who are at risk for developing chronic diseases.

Healthy Living programs and services are offered to patients/clients and VCH staff. We also work with different neighborhoods and groups in Vancouver. Special consideration is given to adults living with health disparities including:
