Home Health Dietitians provide clinical nutrition services to adults 19 years of age or older in your home or at the community health centre as ambulatory nutrition care. Our services range from providing nutrition care to clients recently discharged from hospitals, to nutrition counselling for various conditions using individualized diets devised and monitored by a registered dietitian (medical nutrition therapy). We work with an inter-disciplinary team of case managers, doctors, nurses, and allied health professionals such as speech language pathologists, occupational therapists, and physiotherapists.

Home health dietitians are involved with clients who are unable to leave their home. Clients who wish and are able to be seen in their community health centre may be referred to the ambulatory home health dietitian.
All nutrition services are free of charge.
