We are an interdisciplinary team with a public health nurse, a physiotherapist and an occupational therapist who provide services to Richmond community dwelling older adults and their families.
The program is equally dedicated to direct care and to building community capacity around fall prevention.
A population health approach to provide education and resources for active seniors and their families, community groups, and groups working with/for seniors
Identify for active seniors their risk of falling, fall hazards in their home, and their strength & balance status
Make recommendations for ways to reduce risk of falls
Refer to other health professionals or community services
