Chronic Disease Management, Care Coordination for Patients with Complex Health Needs

In-patient respiratory education for staff and patient at Lions Gate Hospital
Community-based Respiratory Disease Education Program (COPD and Asthma)
One-on-one Self-Management Support for up to 3 months
Pulmonary Rehabilitation ? BREATH Program at Lions Gate Hospital
5-week Group Education and Exercise program
Cardiac Rehab and Cardio Metabolic program ? Service fees apply.
