The North Shore Diabetes Foot Care Program's mandate is to prevent wounds/foot ulcers and lower-extremity amputations.
Clients with diabetes (no open wounds) are seen by a Foot Care Nurse, Occupational Therapist and receive education in self-care home management to maintain healthy feet.
Program includes assessment, education, treatment and recommendations as required. Foot wear and/or orthotics assessed. Once treatment plan is completed, clients are discharged with referral to other health care providers or to public services as needed for follow-up.
Physician referral required. Fee for visit.
