We focus on supporting clients in the prevention of diabetic foot complications, wounds and amputations.
Clients are seen by a Foot Care Nurse and Occupational Therapist for 1-4 visits over a year.
The program includes assessment (e.g. foot wear, orthotics), education, treatment and recommendations as required. Once a treatment plan is completed, clients are discharged with referral to other health care providers or to public services as needed for follow-up.
Any health care provider or physician may refer. Clients with open wounds are ineligible. Fee for service.
