North Shore Pediatric Resource Team - West Vancouver Community Health Centre

?Services for children and youth with special needs on the North Shore are offered by a team of Nursing Support Services Coordinators and Community Pediatric Physiotherapists.

Nursing Support Services include:

  • Coordination and consultation
  • Assessment to determine if Delegated or Direct Care is required
  • Development of Individualized Care Plans
  • Teaching/training and delegation for alternate caregivers
  • Facilitation of the At Home Program
  • Counseling/Referral
  • Monitoring and Evaluating

Early Intervention Physiotherapy

Physiotherapy services for children 0 to 6 years including flexibility to participate in the kindergarten transition year.

Consultation is provided in multiple community settings including:
  • Home
  • Daycare
  • Preschool
  • Kindergarten
  • Infant Development Program.

Physiotherapy for School Aged Children

Physiotherapy serves school-aged children with special needs to achieve optimal independence and to assist them in meeting their educational goals.

The therapist provides consultative service for family members, teachers and teaching assistants to support the student?s participation, inclusion and access to learning opportunities with the goal of achieving the student?s highest level of independent functioning.

Services are provided in home, school, child care and community settings.

There is no charge for our services.
