?We offer practical support to Aboriginal persons utilizing Vancouver Coastal Health services through hospital liaison and referral processes. Our purpose is to provide a communication linkage between the patient, family, community and health care provider, assisting the care team with health care and discharge planning.
Our goal is to improve access and ensure the Aboriginal patients' health care experience is culturally safe and inclusive. We aim to ensure the Aboriginal patient understands the hospital cultural/processes and health care staff understand the unique needs of the Aboriginal patient.
Monday - Friday, 9 a.m. - 5 p.m
Referrals are accepted from the following: self, family, health care provider, social worker, acute and community service provider. The Aboriginal Patient Navigator (APN) program will work with patients in Vancouver who are visiting from other areas of the province.
To refer a patient to the Patient Navigator Program, fill out the Aboriginal patient navigator intake form and email or fax it to the program.
Serves people who self-identify as being Aboriginal or First Nations.
