Providing care close to home

Our primary health care services focus on helping you stay healthy by providing care and treatment, illness prevention, health promotion and chronic disease management services close to where you live.

If you receive information and care close to home, it is easier for you and reduces unnecessary stays in hospitals and residential facilities. Primary care is typically provided by family physicians, nurses, dietitians, mental health professionals, pharmacists, therapists and others.

Examples of primary health care services:

  • Medical clinic visits (when you visit your doctor)
  • Outpatient clinics (when you go to a clinic for follow up treatment and monitoring after an illness or injury)
