Supporting residents with resources to self-manage their chronic conditions
Our partnership with the Richmond Public Library (RPL) on the Richmond Health Literacy Project offers direct support to promote informed choice, involvement and self-responsibility.
Improving the experience of hip and knee surgery patients
We asked patients to provide feedback about the care they received before and after hip or knee replacement surgery. Find out about the improvements that have been made as a result of this feedback.
Reshaping palliative care
In 2005, we led a public consultation about palliative care in four languages across the areas served by Vancouver Coastal Health. Read the report to find out what changes were implemented as a result of this consultation.
Providing online tools for people living with dementia
We collected feedback from people living with dementia and their caregivers and used this information to develop a patient-focused website, the Dementia Journey. Learn more about this website.
Transforming seniors care
We explored how patients, families and the public can work together to improve health outcomes for seniors who enter hospitals. See the tools, a video Dorothy's Story: Seniors, Families, and Professionals - Partners in Care and a brochure, that were developed as a result of this project.
