What makes our communities healthy?

The health of our communities is influenced by the social, environmental and economic conditions in which we live. These are often called the social determinants of health and include factors such as: education, income, supports during early child development and access to safe and affordable housing.

Population health is an approach to health that aims to improve the health of our communities through action on the social determinants of health.

Strategies for population health

We have outlined four strategies for population health:

  1. Leadership
  2. Partnerships
  3. Policy development
  4. Advocacy

Our team

Reporting to the chief medical health officer, our Population Health Team works with a variety of partner organizations to influence the creation of healthy public policies that support the health and well-being of our communities.
