Helping Aboriginal people access health servicesThe Aboriginal Patient Navigator program helps Aboriginal people access health services at Vancouver Coastal Health. Navigator services Patient navigators provide referral, advocacy and support to patients to ensure access to appropriate health care and community services. Their assistance ranges from helping a patient get prescription drug plan coverage to escorting patients to medical appointments to making recommendations for, and assisting with, discharge planning. Navigators typically meet face-to-face with patients and their families. Patients can be referred to a navigator by their doctor or health care provider or patients can access them directly.
