New City Centre Community Access Centre slated for the West End
VANCOUVER – Vancouver Coastal Health (VCH) is moving forward with plans to open a new City Centre Community Health Access Centre in early 2018 dedicated to improving community health services in the West End.
The centre will include the services currently offered at Three Bridges Community Health Centre and by the West End Mental Health team, all consolidated at one location at 1128 Hornby St. The larger space will be open daily, allowing more clients access and better coordination of integrated community care.
“The health system works best when clients have access to care that is integrated and convenient, meeting all of their needs,” said Health Minister Terry Lake. “By expanding to a larger space, the team at City Centre Community Health Centre will be able help even more people in the community.”
Clients will be able to access primary care and new services such as outpatient clinics for IV treatments and wound care at the new community health centre. In addition, the ample space will provide a base for community outreach teams.
“We have been bursting at the seams at the other sites. In this larger space, we can streamline care by bringing together the expertise of all the programs and services currently at Three Bridges and West End Mental Health Team, and provide integrated clinical care in one convenient location,” said Laura Case, VCH Chief Operating Officer.
The increased space means that some programs such as mental health and outpatient clinics that have had limited capacity due to cramped quarters at the other sites will be able to expand and serve more people in need in their home community of the West End.
The new centre will have reception areas on all for floors, wheelchair access, areas for outpatient IV and wound care clinics, expanded room to offer group programming, and space designed for family centered programs.
Transition plans will be put in place so that client care is not disrupted.
Three Bridges Community Health Centre services that are moving to the new locale include Public Health (mom and baby, immunization, speech language clinics), Home Health, primary care, STOP HIV, addiction counselling and Prism (for LGBTQ2S clients). West End Mental Health services include Child and Youth Mental Health, and Adult and Older Adult Mental Health.
The new centre is part of enhanced and expanded services in the West End/Downtown community that have come about after consultations with staff, clients, community stakeholders and residents related to the St. Paul’s Hospital relocation.
