Home Health Dietitians provide clinical nutrition services to adults 19 years or older in the home or at a community health centre as ambulatory nutrition care.

Home health dietitian services range from providing nutrition care to clients recently discharged from hospitals to nutrition counselling for various conditions using individualized diets devised and monitored by a registered dietitian (medical nutrition therapy).

What to expect

Upon admission, all clients 19 years of age and older are screened and detected for nutritional risk. If at nutritional risk, the client will be referred to one of our dietitians for timely intervention and monitoring in the home. 

In addition to mitigating malnutrition, dietitians support clients for reasons such as:

  • medical nutrition therapy,
  • nutrition support (enteral nutrition),
  • surgical recovery,
  • food insecurity and access,
  • chewing and swallowing problems (i.e. dysphagia),
  • poor wound healing, or
  • arranging community supports.
