The Community Rehabilitation and Resource Team provides clinical consultation, rehabilitation and specialized support to adults with complex disabilities.

Clients seen by the team require community-based rehabilitation and/or support in transitioning to the community. These services are designed for clients whose community rehabilitation needs cannot be met by services provided by a home health team.
