Intensive Case Management Teams (ICMT)

Intensive Case Management Teams (ICMT) provide a support role for people who experience severe substance abuse problems, chronic unmet medical needs, and multiple barriers for accessing services.
Women’s ICMT
We have two intensive case management teams (i.e. Heatley ICMT and Women's ICMT) that support people who live in the Downtown Eastside (DTES) in Vancouver who experience severe substance problems and unmet chronic health issues.
The Women’s Intensive Case Management Team’s mandate is to work with marginalized women (Trans inclusive). The team accepts referrals from DTES-partnered women’s organizations and housing organizations that work with marginalized women not connected to health services and meet the mandate of ICMT.

Heatley Intensive Case Management Team
Referrals: Heatley ICMT accepts referrals from VCH partners and St. Pauls’ Hospital.
