Community Transition Teams work with hospital staff to help discharge-ready mental health consumers transition back to the community from an acute psychiatric unit.

Services include

  • assistance with housing,
  • rapid crisis response,
  • connecting clients with community-based mental health teams,
  • monitoring client mental health and medication,
  • as well as assistance in accessing community resources and other health services.

The program also provides short-term crisis-response to adults with serious and persistent mental illnesses.
