<div class="ExternalClass1B8376C48FBC4938A61D9D535FBFECBD"><div><p>The Adolescent and Young Adult Program provides adolescents, age 12 to 19 with acquired or congenital disabilities living in British Columbia and the Yukon an opportunity to address their rehabilitation needs and the transition tasks of adolescences.</p><p>This program provides interdisciplinary assessment and treatment for persons with complex neuro-muscular-skeletal conditions. Adolescents with primarily developmental intellectual impairment or mental health conditions are not seen.</p><div><ul><li>Concussion</li><li>Moderate to severe brain injury</li><li>Spinal cord injury and spina bifida</li><li>Cerebral palsy</li><li>Neuro-muscular disease such as Duchenne's Muscular Dystrophy</li><li>Other congenital or acquired neuro-muscular diseases</li></ul>Services provided:<br><ul><li>Inpatient and outpatient rehabilitation services</li><li>Individual goal setting</li><li>Assessment and treatment</li><li>Assistance with service funding options</li><li>Information and referral to community resources</li><li>Education for clients, families, care providers and schools</li></ul>Team members:<br><ul><li>Physiatrist (Physical Medicine and Rehabilitation Physician)</li><li>Occupational Therapist</li><li>Physiotherapist</li><li>Social Worker</li><li>Speech Language Pathologist</li><li>Teacher</li><li>Recreational Therapist</li><li>Neuropsychologist</li></ul></div></div></div>4255 Laurel Street(604) 730-7904(604) 737-6291V5Z 2G9BC<div class="ExternalClassFA835E8827E14774A5459AB540E8975A"><p>Paid parking is available.
