The Intensive Case Management (ICM) Team provides a support role for people who experience severe substance abuse problems, chronic unmet medical needs, and multiple barriers for accessing services.

How to access

  • Check eligibility

    • Residents of Pemberton or the surrounding areas aged 19 and over with problematic substance use concerns.
  • Contact us

    • Contact the Assessment & Treatment Matching Team at (604) 698-5861 to learn if this service is right for you or for more information.
    • Physician or nurse practitioner referrals are also accepted. If you are a physician or nurse practitioner looking to refer, please fax a completed Sea to Sky Referral Form to 604-894-6491.

The Pemberton Health Centre provides 4 stretchers for emergency care as well as services that include emergency, diagnostic imaging, laboratory and physiotherapy services.
