Intensive Case Management Teams (ICMT) provide a support role for people who experience severe substance abuse problems, chronic unmet medical needs, and multiple barriers for accessing services. Services are offered on a completely outreach basis and the goal is to stabilize clients and connect to appropriate long-term services.

We have two intensive case management teams (i.e. Innercity ICMT and Women's ICMT) that support people who live in the Downtown Eastside (DTES) in Vancouver who experience severe substance problems and unmet chronic health issues. 

Women’s Intensive Case Management Team (Women's ICMT)

The Women’s Intensive Case Management Team works with marginalized women (Trans inclusive). The team accepts referrals from DTES-partnered women’s organizations and housing organizations that work with marginalized women not connected to health services and meet the mandate of ICMT.

Referrals: Women’s ICMT accepts referrals from partnered community organizations.

Innercity Intensive Case Management Team

Referrals: Innercity ICMT accepts referrals from VCH partners and St. Pauls’ Hospital.

Heatley Community Health Centre brings existing programs and services together so clients get the care they need at one location. Primary care, mental health and substance use services, harm reduction and specialized care are all available on-site.
