Get your health records from a hospital or community clinic

Request Health Records

To request information from your health record, you will need to:

  1. Complete the first form below called, “Authorization for the Release of Health Records”.
  2. Fax or mail the completed form to the specific hospital/facility you are requesting records from. Refer to the second PDF below called, "Hospital Contact List" for fax numbers and addresses.

Is there a fee?

This service is free-of-charge. 

When can I expect a response?

You will receive a response within 30 business days from the date we receive your written request. If we need to consult with others or if we have a large volume of records to review, the time may be extended, as permitted under the BC Freedom of Information and Protection of Privacy Act. The E-Health (Personal Health Information Access and Protection of Privacy Act) is also available for your reference. 

If there is a delay, we will notify you in writing of the reason for the extension and the new due date.

Contact Health Records Services at the location you were treated and ask for the Release of Information department. The contact information for our locations is listed in the Find Locations section.
