Case management services for clients 19 and over who present with a serious and persistent mental health disorder with or without a substance use disorder that is significantly affecting their functioning.

    • Ages 19 and older
    • Experiencing mental health concerns both with or without substance use concerns

    • Physician or nurse practitioner referrals are also accepted. If you are a physician or nurse practitioner looking to refer, please fax a completed Sea to Sky Referral Form to 604-894-6491.

Pemberton Health Centre

The Pemberton Health Centre provides 4 stretchers for emergency care as well as services that include emergency, diagnostic imaging, laboratory and physiotherapy services.

Adult Mental Health Teams

Community-based assessment and treatment for adults who are diagnosed with a serious and persistent major mental disorder and experiencing significant problems that interfere with their functioning in daily life.
