The BREATH North Shore Pulmonary Rehabilitation Program supports your ability to self-manage your chronic lung condition through learning, exercising and connecting with others.

How to access

  • Our program

    The eight-week program consists of:  

    • Initial assessment with the program team/respirologist
    • 16 Group Education sessions covering a variety of topics, including:
      • Lung conditions
      • Chest infections
      • Breathing control
      • Bronchial hygiene
      • Pulmonary function tests
      • Oxygen therapy
      • Drug delivery systems
      • Exercise
      • Action plan
      • Nutrition
      • Energy conservation
      • Relaxation techniques
      • Stress management
      • Coping skills
      • Intimacy
      • Travel planning
      • Self-management
      • Goal setting
    • Each group education ends with an exercise component which includes aerobic exercise, weight training and stretches.
    • A follow-up assessment at 3-6 months after the completion of the program is provided.

    Sessions run from 1 to 2:30 p.m. every Tuesday and Thursday for eight consecutive weeks. We have 10-12 participants in each session. Nearing the end of the program, the program team will connect participants to other programs offered in the community so they can keep active and exercise safely.

    A doctor or nurse practitioner must provide a referral with copies of requested tests before appointments can be booked for the next available assessment program.

    • Get a referral

      Any doctor or nurse practitioner may refer. Have them complete the North Shore Chronic Disease Service referral form and fax the completed form to (604) 297-9681.

      The referral form must include copies of the requested tests before the appointment can be booked. The tests include:

      • Medical history (within one year)
      • Pulmonary function (within three years)
      • ECG (within six months)
      • Chest x-ray (within one year)

    Parkgate Community Health Centre

    ​The Parkgate Community Health Centre opened in 1999. In addition to providing a wide range of health services and supports, it also provides clients with easy access to a range of other community services, including the library, seniors centre, and a variety of recreational programs for all ages.
