Chronic Disease Management at North Shore Chronic Disease Services

Chronic disease management services assist adults with chronic conditions and difficulty managing their health or who want to know more about their condition.

How to access

  • Check eligibility

    ​Must be North Shore residents with 2 or more chronic diseases and has complex high needs.   

  • Get a referral

    Referrals from a Physician or a Nurse Practitioner are accepted. Download the North Shore chronic disease referral form.

  • Monday:   8:30 a.m. to 4:30 p.m.
  • Tuesday:   8:30 a.m. to 4:30 p.m.
  • Wednesday:   8:30 a.m. to 4:30 p.m.
  • Thursday:   8:30 a.m. to 4:30 p.m.
  • Friday:   8:30 a.m. to 4:30 p.m.
  • Saturday:   Closed
  • Sunday:   Closed

Parking & Transportation

​Underground parking is available, as well as limited street parking. 
Elevators are available from the underground parking lot.
