There are two services in this location: the Geriatric Rapid Access Clinic and the Teaching Clinic.

How to access services

  • Geriatric Rapid Access Clinic

    We provide comprehensive geriatric physiotherapy and occupational therapy assessment and treatment.

    Our program is intended for patients greater than 65 years old and suffering from dementia, delirium, disruptive behaviour, incontinence and failure to thrive as well as those with increased risks of institutionalization.

    Hours of operation

    Tuesday, Wednesday and Friday from 8 a.m. to 4 p.m.

  • Teaching Clinic

    This clinic provides a geriatric medicine specialist assessment of frail older adults with complex medical, psychiatric, social and functional issues.

    Eligibility and referral

    This service is for seniors. Common reasons for referral include:

    • Cognitive impairment          
    • Multiple medical comorbidities and frailty
    • Functional decline (ADLS, IADLS concerns)
    • Pre-operative assessment
    • Mobility concerns and fall assessment
    • Delirium
    • Medication review/polypharmacy
    • Behaviour/mood concerns
    • Movement disorders (Parkinsonism, etc.)
    • Syncope, fainting
    • Living at risk/failure to thrive
    • Competency assessment

    Monday to Friday, 8 a.m. to 6 p.m.

The Geriatric Rapid Access Clinic is open Tuesday, Wednesday and Friday from 8 a.m. to 4 p.m. The Teaching Clinic is open Monday to Friday from 8 a.m. to 6 p.m.
