If you or someone you know is interested in receiving home and community care services, call the access line contact information in your area.
When you connect with the home and community care office, a Home Health clinician will help you determine eligibility and if appropriate, arrange to conduct a health needs assessment.

Hours of operation
We operate 24 hours a day, 7 days a week
This care home provides 24-hour nursing care.
Visiting hours are Monday - Friday, 9 a.m. to 5 p.m.
Parking and transportation
Situated right at the edge of Chinatown and opposite to the Dr. Sun Yat-Sen Garden, the care home is very much connected to the neighbourhood where shops, eateries, banks and supermarket are available.

Operated by S.U.C.C.E.S.S. Multi Level Care Society, this long-term care is a three-storey concrete building with five resident units – Pine Garden, Bamboo Garden, Cherry Garden, Birch Garden and Cedar Garden.

The S.U.C.C.E.S.S. Simon K.Y. Lee Seniors Care Home is a three storey building in Chinatown with five resident units – Pine Garden, Bamboo Garden, Cherry Garden, Birch Garden and Cedar Garden. The ground floor includes an Adult Day Programs, and a Special Care Unit with 23 beds. Four other resident care units occupy the second and third floors.
