When you connect with the home and community care office, a Home Health clinician will help you determine eligibility and if appropriate, arrange to conduct a health needs assessment.

- Friday: 9:00 a.m. to 5:00 p.m.
- Saturday: Closed
- Sunday: Closed
This facility provides 24-hour nursing care.

There are a few places to park at the front of the building, some street parking and a parking lot at the back of the building accessible from 33rd.

The second floor is a special care neighborhood for people living with dementia. This long-term care home is operated by Providence Health Care.
