HealthConnection provides primary care for North Shore residents who don’t have a regular family doctor or nurse practitioner and who have complex medical, mental health and substance use, and/or socio-economic needs such as housing, income or access to food.

About this service

The clinical team at HealthConnection includes Doctors, Nurse Practitioners (NP), Chronic Disease Nurse (CDN), Social Services Support Liaison and Clinical Assistants. 

Our community partners (Canadian Mental Health Association, Hollyburn Family Services, Lookout Shelter, and North Shore Neighbourhood House) can provide outreach and to help with your non-medical needs.

The Health Connection Clinic is made possible through a partnership between the North Shore Division of Family Practice and Vancouver Coastal Health.

  • Thursday:   9:00 a.m. to 12:00 p.m., 1:00 p.m. to 4:00 p.m.
  • Friday:   9:00 a.m. to 12:00 p.m., 1:00 p.m. to 4:00 p.m.

Monday to Friday, 9 a.m. to 12 p.m. for drop in. Monday to Friday, 1 p.m. to 4 p.m. by appointment

Entrance to the building is set back off the street. ​Street parking is available.
