A referral is required. Have your care provider complete a referral form and fax the completed form to (604) 984-3766.

- Tuesday: 8:00 a.m. to 5:00 p.m.
- Wednesday: 8:00 a.m. to 5:00 p.m.

- Friday: 8:00 a.m. to 5:00 p.m.
- Saturday: Closed
- Sunday: Closed

Lions Gate Hospital is located on 231 East 15th Street in North Vancouver.
Paid visitor parking is available in the hospital parkade on 15th Street, east of St. Georges Avenue.
