We are an interdisciplinary team with a public health nurse, a physiotherapist and an occupational therapist who provide services to Richmond community-dwelling older adults and their families. The program is equally dedicated to direct care and to building community capacity around fall prevention.

How to access

  • Check eligibility

    Clients must meet the following requirements:

    • Age 65 years or older
    • Richmond resident
    • Community dwelling
    • Medically stable
      • Health condition is well-controlled and managed with routine monitoring
      • No medical issue that is acute and/or currently requiring further medical investigation
    • Functionally independent (Clinical Frailty Scale 1-5)
    • Cognitively intact (MMSE>24)
    • Can direct own care and follow direction
    • Provides consent for referral and is open to recommendations
    • Not palliative
    • Not currently involved with another VCH Rehab Program or insurance claim related to referral (ICBC, Work Safe, 3rd party insurance)
    • Level 4 or 5 priority rating by Continuing Care – less urgent/lower risk

How to get there

Enter through doors to the building lobby and check in at reception in the lobby. 

Parking

  • Access to the Parkade driveway only available when driving East on Granville Avenue

  • Pay parking available on first two levels of the parkade – purchase 2 hours of parking

  • Pay machine on 1st level $6.25 for 4 hours – accepts coins or credit card.
