Palliative care is one of the many home and community care services that Richmond Home Health offers. Palliative care can be delivered in outpatient community clinics and in your own home. There are two outpatient community clinics – the specialized Richmond Pain and Symptom Management Clinic and the Home Health Ambulatory Community Clinic. The frequency of your appointments will depend on your care needs. If you are having challenges or concerns in attending outpatient appointments, please let your community team know as soon as possible, the community care team will transition to visiting you in your own home to provide palliative care.

You can contact the home and community care access line to arrange for care or have a health-care professional make a referral on your behalf.

What is the difference between the Richmond Pain and Symptom Management Clinic and the Home Health Ambulatory Community Clinic?

The Pain and Symptom Management (PSM) clinic works as a consulting service for patients who are able to attend an outpatient clinic setting. The clinic collaborates with your family doctor and other specialists to improve your quality of life by managing symptoms such as pain, shortness of breath, and nausea. Your family doctor, oncologist, or home health nurse can refer you to this clinic. In the clinic, you will be meeting with a palliative physician and a palliative Clinical Resource Nurse (PCRN). Social work, occupational therapy, physiotherapy, and a registered dietitian may be referred to you once you are involved with the PSM clinic. 

The Ambulatory Community Clinic is part of the Richmond Home Health program, where patients can receive nursing support, such as wound care, IV management, and palliative care. During your clinic appointment, you will be meeting with a community nurse who can provide physical and emotional support and can refer to other care team members as needed. Nurses will support you to manage physical symptoms that you may be experiencing and to answer any questions you might have about your health journey.

A palliative approach to care can be provided to you and those you choose to be involved in your care from the comfort of your home or assisted living home home.
