The ALS and Related Disorders (ALSRD) Clinic provides ongoing assessment, intervention, consultation and educational services to people with ALS and related motor neuron diseases (MNDs).
How to access
We encourage referral to the ALS and Related Disorders Clinic for all patients where the referring doctor believes ALS/PLS to be highly likely. Patients do not need to have a definite diagnosis of ALS or PLS before referral.
Referrals must be made by a physician or nurse practitioner. Have your physician or nurse practitioner complete the Amyotrophic Lateral Sclerosis (ALS) and Related Disorders Program referral form and fax the completed form to 604-822-2611.
