Home Health Dietitian services range from providing nutrition care to clients recently discharged from hospitals to nutrition counselling for various conditions using individualized diets devised and monitored by a registered dietitian (medical nutrition therapy).

Check eligibility
Home Health Dietitians work with clients who are unable to leave their homes. The community health team may refer clients to the ambulatory Home Heal Dietitian if they can and are willing to be seen at the community health centre.
All nutrition services are free of charge.

Get a referral
If you are an existing client of a community health team, please speak to your Vancouver Coastal health contact and ask to be referred to a Home Health Dietitian.
External referrals can be made through your primary care provider.

- Monday: 8:00 a.m. to 5:00 p.m.
- Tuesday: 8:00 a.m. to 5:00 p.m.
- Wednesday: 8:00 a.m. to 5:00 p.m.
- Thursday: 8:00 a.m. to 5:00 p.m.
- Friday: 8:00 a.m. to 5:00 p.m.
- Saturday: Closed
- Sunday: Closed
