Home Health Dietitian services range from providing nutrition care to clients recently discharged from hospitals to nutrition counselling for various conditions using individualized diets devised and monitored by a registered dietitian (medical nutrition therapy).

How to access

  • Check eligibility

    Home Health Dietitians work with clients who are unable to leave their homes. The community health team may refer clients to the ambulatory Home Heal Dietitian if they can and are willing to be seen at the community health centre.

    All nutrition services are free of charge.

  • Get a referral

    ​If you are an existing client of a community health team, please speak to your Vancouver Coastal health contact and ask to be referred to a Home Health Dietitian.

    External referrals can be made through your primary care provider or by calling Central Intake at (604) 263-7377.

Parking & transportation

There is free on-site parking on the north and south sides of the building.

South Community Health Centre is accessible by public transit. Take the #49 bus to Knight Street or the #22 bus to East 49th Avenue.

South Community Health Centre

South Community Health Centre provides a range of health care services in a single location, including access to public and community health nurses, mental health and addiction counsellors, dental clinics for children, speech therapists, nutritionists, youth drop-in health clinics and more. We see people of all ages who live in South Vancouver (Community Health Area 6).

Home Health Dietitians

Home Health Dietitians provide clinical nutrition services to adults 19 years or older in the home or at a community health centre as ambulatory nutrition care.
