Harm reduction: Toward the Heart 

Nurses, community outreach workers, and psychiatrists provide services to adults with serious and persistent mental health concerns. These services focus on assessment, case management, clinical intervention, support, advocacy, rehabilitation, and community integration.

What these services include

  • Mental Health service coordination, case management, assessment, and treatment planning
  • Psychiatric consult, treatment, and supports
  • Psychiatric medication prescription and management
  • Referrals and linkages to other supports and healthcare resources

Vocational Work Program

Working in partnership with the Adult Community Support Services, the Vocational Work Program helps people gain experience, skills and confidence in preparation for employment.
