Assisted living services provide housing, hospitality, and personal care services for adults who can live independently and make decisions on their behalf but require a supportive environment due to physical and functional health challenges.

When you connect with the home and community care office, a Home Health clinician will help you determine eligibility and if appropriate, arrange to conduct a health needs assessment.

Features

Operated by the Salvation Army, Southview Heights is a 3-storey, wood frame building built in 2006-2007.

Room and building: There are 53 subsidized assisted living one-bedroom units. They are between 558 and 581 square feet. Each suite includes a kitchenette, with full size refrigerator and stove, living room, balcony or patio, separate bedroom and personal bathroom. Floor plan of a typical suite at Southview Heights 

All suites are equipped with a stove and refrigerator. Residents are to provide their own furnishings. Comfortable furnishings are provided in both the dining area and common amenity space. 

A tuck shop and hairdresser are available for tenant use. There is a library, computer lab with internet access and games room located in the core section of the building. There is also a Wellness Centre and an Ambulatory Care Clinic available for tenant and community use.

Language, culture, religious affiliations: The Salvation Army Southview Heights provides dignity, choice and independence in a community built on Christian principles for senior adults requiring supported living.

Accreditation status: Certificate of Accreditation in accordance with The Salvation Army Social Services Accreditation Standards and Review Procedures.

Accreditation is not a requirement of assisted living but some residences have received accreditation status. It is an external peer review process to assess and improve the services health care organizations provide to their residents, based on standards of excellence set by Accreditation Canada

Philosophy of care: Assisted living provides accommodation, hospitality and personal care services in a manner that:

  • Promotes and maximizes choice, dignity, and control for Residents;
  • Promotes the Residents’ right to privacy as well as opportunities for social interaction;
  • Supports Residents’ independence through physical design features in the building and the delivery of support services;
  • Provides care and services that are shaped to meet the Residents’ needs; and
  • Creates a home-like environment that supports the Residents’ preferred lifestyle 

Food service: Table service will be provided in the main dining room at Lunch and Supper, with a minimum of 2 alternates for each meal. Special diets available upon request. Snacks will be available at designated times. 

Special recreation opportunities: Social/Recreational activities are available on a daily basis as well as spiritual & religious care and health maintenance activities.

Accessibility: Residents must be able to move around their suite on their own, and make their way to the dining room, with or without mobility aides. Scooter parking in garage area only. No scooters permitted in main resident areas.

Safety and security: The building is fully equipped with sprinklers, including common areas and resident suites. 

Smoke detectors are located in all rooms, elevators and common areas. Main door is secured at all hours. Visitors ring tenant’s suite to gain access to the building. 24-hour on-site staff. Each suite is equipped with a 24-hour response system, which is monitored by on-site staff. Included with the system is a pendant for personal use.

Neighbourhood and community: Located next door to the Champlain Mall (Bank, grocery, dry cleaning, restaurants, hair dresser, market). City bus stops just outside Southview on Kerr St. The Fire department is one block away. Champlain Heights Recreation Centre also in close proximity.

Exit criteria: Not able to accommodate individuals who are not able to make the range of decisions necessary to function safely in Assisted Living or who require 24-hour professional care. Residents will be supported to find appropriate alternate arrangements.
