G.F. Strong Adult Concussion Services (GFACS)

G.F. Strong Adult Concussion Services (GFACS) provides group education sessions to help adults recover from concussions and a rehabilitation program for people who have prolonged concussion symptoms after returning to their primary care provider.
What to expect
The GF Strong Adult Concussion Services (GFACS) offers two streams of service:
- GFACS Group Education Session
- GFACS Rehabilitation Program
To qualify for the GFACS Rehabilitation Stream, clients must be able to participate in a self-management rehabilitation service. This means they should be able to independently schedule appointments, actively participate in 50-minute therapy sessions (either in person or virtually), and be independent in their self-care activities. Clients can benefit from this program if they can identify their functional goals and are willing to work on practical action plans and exercises between therapy sessions.
GFACS service model
- Clients require a referral to the GFACS: Group Education Session.
- Following the group education session, clients return to the care of the family physician or medical clinic. The primary care provider is encouraged to prioritize assessing and treating depression/anxiety, sleep, and headaches in individuals following concussion as per Ontario Neurotrauma Foundation (ONF) 3rd Edition guideline for concussion/mild TBI.
- Clients who attended the GFACS: Group Education Session and continue to have prolonged concussion symptoms are eligible to be referred to the GFACS: Rehabilitation Program.

Check eligibility
- Adult ages 18 and over living within Live within the VCH health authority region.
- Diagnosis of concussion/mTBI less than one year ago
We do not offer service for active ICBC or WCB clients

Get a referral
Referrals must be made by a physician or nurse practitioner. For questions on referrals, call 604-737-6458.
Have your physician or nurse practitioner complete the GFACS: Group Education Session Referral Form and fax the completed form to the Intake Office at 604-730-7904 or by email to gfsadultconcussionservices@vch.ca.
How to access the GFACS Rehabilitation Program
Clients who attended the GFACS: Group Education Session and continue to have prolonged concussion symptoms may be referred to GFACS: Rehabilitation Program.

Admission criteria for the Rehabilitation Program are the same as the Group Education Session.

A referral form for the Rehabilitation Program is provided to the primary care provider following the client's attendance at the GFACS group education session.
