Sumac Place is a Tertiary Mental Health facility that delivers specialized mental health rehabilitation and residential services for adults who need a more extended, 24-hour supportive care environment.
How to access
Your health care professional will complete a referral form. The Tertiary Mental Health and Substance Use Central Access and Discharge team will review the information. A committee of experts from across the VCH/PHC region will discuss and decide which tertiary program is right for you at this time.
When a space is available in the designated program, the Tertiary Mental Health and Substance Use Central Access and Discharge team will contact the health-care professional who completed the referral. Plans will be made with you and your family to make your transition to the program as smooth as possible.
