Transcript requests and training verification

VGH School of Nursing – Transcript Requests
If you are a former student of the Nursing Diploma Program at The Vancouver General Hospital, your transcript is on file with the Clinical Education Department.
1) Submit online request form
To obtain a copy of your transcript, please submit your request using this online request form. You will need to provide the following details:
- Surname(s) when enrolled as a student
- Current name
- Month & Year of Graduation
- Telephone number
- Email address
- Address where you would like the transcript sent
- Important: If you are requesting an electronic transcript to be emailed to the requesting institution directly, please note that we only provide scanned and email attachments via email. A password-protected link for access is not available.
2) Mail payment
Submit the online request and then mail the payment. Your request will be on hold and your transcript will not be mailed until we receive the payment. We will send a confirmation email to you once the request is mailed.
Cost
- Non-VCH employees: $15 CAD each transcript
- VCH employees: Free

If you are a third party requesting a simple degree confirmation letter via email, please submit the online request form and indicate your company in the comments
