The Youth Home Stabilization Team works with young people (ages 13 to 24) who are using substances and want to stop or significantly reduce the use of at least one substance.

What to expect

This team supports youth and their families who are navigating the first few weeks of recovery while living in community.

For withdrawal support, youths must have a safe place to stay, and it is recommended that they have a support person who will help them during the withdrawal and change process.

For withdrawal of heavy alcohol and/or benzodiazepine use, medically supported in-patient withdrawal management may be required as well.

Short term case management is provided with goal to connect to ongoing recovery based services.

Home Stabilization Team consists of:

  • Team Leader
  • Intake Clinicians
  • Case Manager/Clinicians
  • Youth Outreach Workers
  • Indigenous Cultural Worker
  • Nurse Practitioners

This program is offered to youth residing in Metro Vancouver, and is run by Vancouver Coastal Health.

How to access services

Referrals are required and are completed by a community counselor or health-care professional in collaboration with the client. Once a referral is received, it is reviewed by the Central Addiction Intake Team (CAIT) Concurrent Disorder Counselors for completeness and to ensure clients are matched to the facility best suited to their needs.
